How to choose a natural toothpaste
Choose toothpaste by the job you need it to do, not by whether the front says “natural.” Start with cavity risk and any diagnosed problem, then evaluate fluoride, abrasivity, surfactants, preservation, testing and the evidence behind each claim. Natural origin can be a preference. It is not a safety or performance result.

First, define the job
The most useful first question is not “Is it natural?” It is “What do I need this toothpaste to do?” A person with recent cavities, persistent dry mouth, exposed roots, orthodontic appliances, sensitive teeth or active gum treatment is making a clinical choice. A person comparing flavor, texture, foam or packaging is mostly making a preference choice.
Those categories can overlap, but they should not be confused. Claims such as anticavity, antigingivitis and sensitivity relief concern health outcomes and need evidence for that use. Claims such as mint flavor, oil-based texture or no artificial color describe the product experience. The FDA also classifies a product by its intended use, so the claims around a formula matter as much as the ingredients themselves.1
Treat “natural” as a prompt to look closer
The FDA has not established a formal definition of “natural” for cosmetic labeling. The word can describe sourcing, processing, a brand philosophy or little more than package design. It does not identify which ingredients are present, in what amounts, or how the finished paste performs.
Natural origin also does not guarantee safety. Plants can contain allergens and irritants, while synthetic ingredients can be well characterized and tightly controlled. The FDA’s position is direct: safety depends on the ingredient, dose, formulation and conditions of use, not source alone.2 Read the complete ingredient list and directions instead of using one adjective as a verdict.
Make the fluoride decision explicitly
Fluoride is the best-established anticavity ingredient in toothpaste. Cochrane reviews find that fluoride toothpaste reduces caries compared with non-fluoride toothpaste, and that concentrations of 1,000 ppm and above have clearer evidence than lower concentrations.34 The ADA recommends brushing twice daily with fluoride toothpaste for most people.5
Choosing fluoride-free toothpaste is therefore a trade-off, not a purity upgrade. Someone may still make that choice because of personal preference or advice specific to their situation, but recent cavities, dry mouth, exposed roots, braces, frequent sugar exposure or a prescribed high-fluoride routine are reasons to involve a dentist. Do not assume xylitol, baking soda, hydroxyapatite or any other ingredient is a one-for-one substitute without product-specific evidence.
Ask about the finished formula’s abrasivity
Toothpaste needs enough cleaning action to remove deposits and surface stain without turning daily brushing into unnecessary wear. Relative dentin abrasivity, or RDA, is a standardized laboratory comparison. It is useful for formulation and quality control, but it is not a percentage or a complete prediction of clinical wear.6
The words “natural,” “charcoal,” “whitening,” “baking soda” and “gentle” do not reveal a product’s RDA. Particle size and shape, concentration and the rest of the formulation all matter. If you have exposed dentin, erosion, recession or active tooth wear, ask for a current finished-product result and discuss it with your dentist. Separate foam from cleaning
Surfactants help toothpaste spread, mix with saliva and disperse oily material. They also produce foam, which many people read as a signal that the paste is working. Visible lather is not a direct measure of plaque removal. A systematic review found moderate-certainty evidence that adding dentifrice did not improve the immediate mechanical plaque removal achieved by brushing alone, although toothpaste still delivers ingredients and other functions that a dry brush does not.7
If tolerance or mouthfeel matters to you, distinguish SLS-free from surfactant-free. The first excludes one surfactant; the second describes a broader formulation choice. Neither label proves superiority for every user, and a non-foaming paste still depends on complete, gentle brushing.
Look for a credible preservation strategy
Preservatives are not automatically undesirable. A water-containing product used in a bathroom needs protection against foreseeable contamination. A well-designed preservative system is one way to provide it. A genuinely low-water-activity formula is another, but “water-free” on a label is not itself a test result.
Ask how microbial safety was evaluated on the finished product, whether the test relates to the current formula and packaging, and what storage or use conditions it covers. FDA guidance places responsibility for cosmetic safety and microbiological quality on the manufacturer.89 The preservatives guide explains why low water activity can limit growth without making a product sterile.
Read testing and marketing claims as separate things
A useful test report identifies the laboratory, method, sample or lot, result, units, reporting limit and date. It should be clear whether the work measured one ingredient, a prototype or the finished product. “Dentist formulated,” “lab tested” and “third-party tested” can be informative, but they do not name a result by themselves.
For any performance claim, ask five questions: What exactly was measured? Was the finished formula tested? Was there a comparison group or acceptance criterion? Does the result match the wording on the label? What does the study not establish? The ADA Seal is one independent, claim-specific evaluation route, but its meaning is limited to the claims reviewed for that category.10
A practical decision checklist
- Need: Write down the health outcome or experience you are trying to change.
- Active ingredients: Check whether the formula contains an active supported for that outcome.
- Risk: Put recent cavities, dry mouth, exposed roots and active treatment ahead of lifestyle preferences.
- Whole formula: Review abrasives, surfactants, flavors, sweeteners, carriers and preservation together.
- Evidence: Prefer finished-product data that identify the method, date and sample.
- Use: Choose a flavor and texture you will use correctly for two minutes, twice a day.
- Escalate: Ask a dentist about ongoing pain, sores, bleeding, sensitivity, dry mouth or repeated decay instead of solving a symptom by label shopping.
Where the evidence stops
There is no single clinical trial that ranks every “natural” toothpaste as a category, because the category has no standard formula. Evidence usually belongs to a specific active ingredient, outcome, dose and product. Ingredient-level findings cannot automatically be transferred to a finished paste, and one laboratory report cannot establish every aspect of long-term use.
This framework can narrow the shelf. It cannot calculate your personal cavity risk, diagnose irritation or replace a recommendation based on an examination and history.
